Complete Toilet Flange Failure

Women’s bar restroom - Left toilet - Toilet flange failed and metal
ring snapped off from around the lip of toilet flange. Entire flange
remained glued and intact inside concrete slab. To make repair, full
extraction of ABS pipe/fitting for flange needs to be carefully
removed from inside the hub of the the 90° fitting in the slab to be
able to accept the installation of the new flange and to allow for a
secure seal with ABS glue. Removed all of the existing flange and
installed new flange (with metal ring) and drilled and secured

concrete anchors into flange to maintain a permanent placement
with the inability to bs e same issue as before with flange failing
and causing the toilet to fall over. When setting toilet, a new extra
tall wax ring (with plastic horn) was used and new hardware
(flange/Johnny bolt set nuts/washers). Upon installation, verified a
secure seal with floor and wax ring by completing a full flush 10x in
rapid succession. After verifying zero issues with toilet and
installation, joint at toilet and tile was sealed with white waterproof
caulking. Toilet and floor were swept, mopped and sanitized to pre-
work condition.
