Forwarded herewith is the Tree Cutting Permit No. RXII-078-2019 issued in


your favor by the DENR RXII, Koronadal City with the terms and conditions
stipulated therein relative in the cutting of Ten (10) planted trees of various species
with an aggregate volume of 2.16 cu.m. situated within the premises of Bentung
Sulit National High School. Reiterating the following terms and conditions, to wit;

1. That only those planted trees as indicated above, and no naturally grown
trees therein, should be cut as shown in the table below:

2. That in the cutting of the aforesaid trees, the permittee should exercise
extra care so as to avoid accidents e.g. injury and damage to lives and
properties;

3. That the permittee shall undertake measures during and after tree cutting
operations to mitigate the possible impacts (e.g. erosion) of the said
activity to the locality and to the environment;

4. That any issue that may arise from the cutting of the trees shall be the
responsibility of the applicant;

5. That no actual cutting is done unless supervised by the CENRO;


6. That the permittee shall replace each tree to be cut with at least fifty (50)
seedling all preferably of indigenous trees species to be planted by the
permittee in coordination with DENR Region XII, Koronadal City or his
duly authorized representative and to be planted in an area agreed upon
by both parties in support of the National Greening Program (NGP) and
Climate Change initiatives of the government. Replacement planting shall
maintain the seedlings planted for a period of three years with 80%
survival rate;

7. That the timber/logs of said ten (10) planted trees of various species shall
be turned-over/delivered immediately after cutting by the permittee to the
DENR-CENRO General Santos City.

8. That for record purposes, you shall submit to the Regional Office
immediately after the cutting, photographs of the said cut trees and copy
of the receipt of turn-over of same to the DENR-CENRO General Santos
City.

9. That this permit is effective from date of issue hereof and expires 50 days
thereafter or automatically as soon as the said ten (10) planted trees of
various species are cut; and

10. Violation of any of the above conditions as well as other existing forestry
rules and regulations shall be sufficient ground for the
cancellation/revocation of the permit without prejudice to the imposition of
other appropriate penalties or sanctions.

the above address when conducting the cutting activity for us to dispatch our
personnel to monitor your compliance on the above-mentioned conditions.
