Forwarded herwith is the Tree Cutting Permit No. RXII-060-2019 issued by the
DENR RXII, Koronadal City with the terms and conditions stipulated therein relative in
the cutting of Twenty five (25) planted trees with an aggregate volume of 8.84 cu.m.
situated within the premises of Dadiangas West Elementary School, General Santos
City. Reiterating the following terms and conditions, to wit;

1. That only those planted trees as indicated above, and no naturally grown
trees therein, should be cut as shown in the table below:

2. That in the cutting of the aforesaid trees, the permittee should exercise extra
care so as to avoid accidents e.g. injury and damage to lives and properties;
3. That the permittee shall undertake measures during and after tree cutting
operations to mitigate the possible impacts (e.g. erosion) of the said activity
to the locality and to the environment;
4. That any issue that may arise from the cutting of the trees shall be the
responsibility of the applicant;
5. That this permit is effective from date of issue hereof and expires 50 days
thereafter or automatically as soon as the said twenty five (25) planted trees
of various species are cut; and
6. Violation of any of the above conditions as well as other existing forestry
rules and regulations shall be sufficient ground for the
cancellation/revocation of the permit without prejudice to the imposition of
other appropriate penalties or sanctions.

above address when conducting the cutting activity for us to dispatch our personnel to
monitor your compliance on the above-mentioned conditions.
