To Whom It May Concern,

I hope this letter finds you well. I am writing to provide an explanation for my absence from work
during the period of January 17 to January 31. I deeply regret any inconvenience my absence
may have caused and seek your understanding regarding the circumstances that led to my
inability to attend work during this time.

On January 16, I obtained approval from the MOD to undertime due to a severe toothache,
causing swelling and hindering my ability to carry out my responsibilities due to the associated
pain.

Subsequently, from January 17 to January 21, I was unable to attend work due to the persisting
severity of the toothache.

From January 22 to 24, I applied for vacation leave and requested a shift in my rest day to the
25th. On the 22nd, I was processing the renewal of my driver's license, and on the 23rd, I
celebrated my birthday with my family. On January 24, an unexpected opportunity arose,
compelling me, along with my partner and parents, to travel to Masbate to seek my partner's
hand in preparation for our wedding. Although I had intended to return to Cebu on the 25th,
transportation constraints forced me to reschedule, as the next available via Bogo ferry trip was
on January 29 since I brought a multicab with me back to Cebu.

Upon my return on the 29th, I reached Bogo Port on the 30th. Unfortunately, on the way back to
Consolacion, the multicab broke down, and I faced challenges in seeking assistance as I was
alone since my partner stayed in Masbate for a few more days, and my parents had to stay as
well for a church activity. Consequently, I was unable to report to work and inform the MOD due
to poor reception and the breakdown.

Regrettably, on January 31, I received a message from the MOD instructing me to report to the
HR office for an explanation of my absences.

On February 1, I returned to work and was advised by my MOD to await the letter that HR
would be sending to me before resuming work, and I should provide my explanation
accordingly.

I acknowledge the inconveniences caused by my absence and assure you that I have
implemented corrective measures to prevent a recurrence. If you require additional information,
please feel free to contact me. I sincerely hope for your understanding and consideration in this
matter.
