Description of Non Conformance: (Image attached) It has been observed that hadn't
conducted the necessary leak test for the oxygen & DA (Gas Cylinder) and
combusitable material placed nearby it.

Description of the problem which requires rectification:

Conduct a site inspection before commencing of any job for ensuring no combustible material is
present in the area.
Verify the gas cutting set's integrity through on-site leak testing prior to starting work.
Site in charge have reviewed and approved the daily checklist.
Replace the NRV if founds any leak while testing and submit the test report.
Provide the effective training to the operator & workers regarding the Gas Cutting check points
Ensure that the Pre-toolbox is been carried out in the premises.
Inspect the site conidition by the Project Manager & Safety Officer before start of the work.

Closure report has to submit with action plan and evidence of improvement.

Action Taken: 1. Effective training conducted to the operator & workers regarding the Gas Cutting
work.
2. Flashback Arrestor replaced and Leakage test conducted.
3. Leakage test conducted & now no leakage found.
4. Combustible materials has been removed from the work area.
