TALENT/ARTIST

AGREEMENT

This serves as an agreeemnt between John Jacob Plaza (“Talent”) and the undersigned “Client”.
Both parties are obligated to accomplish their part of the agreement as per below:

Location : THRU ONLINE

2. Talent Fee
The below talent fee excludes transportation, accommodation and food and will be provided
by the “client”, unless otherwise stipulated below.
The “client” shall provide payment to the“talent” as per below schedule.
3,000 php Paid
Booking fee: Due date :
Balance: 3,000 php Due date : For succeeding 6-13 sessions
The below inclusions will apply if countersigned and agreed by the “talent” :
Transportation : Accomodation :
Food / Refreshments :
The “client” may pay through bank transfer, g-cash or in cash, and must have a proof of
transaction as a back-up of payment for this agreement. Cheque payments may be
accepted, but must be paid in cash immediatey if bounced. Failure between parties may
cause pursuance of legal action and/or publicize the failure of compliance to this contract.
Booking fee is non-refundable, and this agreement will be in effect upon receipt of booking
fee only.
3. Cancellation
Cancellations must be made at least 1 week prior to the activity date/time. Failure to cancel
in this given time frame shall obligate the “client” to pay the “talent” 100% of the Talent Fee.
Exemption would be natural calamity/disaster or illness of the talent or the client that may
affect the event/activity. In this occurence, both parties may either agree to re-schedule,
refund or end the contract without payment of the balance.
