Service Report/ Checklist The Contractor must prepare and sign two

TREATMENT OF
The service contractor shall apply water treatment to the cooling

 one copy of the report shall retained by the Technical Officer. A sample
of the service water treatment report is attached for reference.
UNSCHEDULE Months 1 8
MAJOR
All unscheduled major overhaul, minor repair, as per request

ii) Major Overhaul/Repair/Replace Major overhaul such as breakdown
of chilled water pump set needed works for allignment shaft, replace
mechanical seal, replace bearing, major overhaul compressor unit for
AHU needed to be dismantle on site and replace with new
piston/terminal and all other related parts with component Water
cooled package unit system and all other related major works
regarding to Water cooled system.
iii) Minor Repair/ Replace/ Rectify Minor repair/replace and rectify
including for all water cooled package which is AHU/FCU/ACSU,
control panel for water cooled system system of cooling tower, chilled
water pump set, AHU and FCU such as replacement of timer, relay,
molded case circuit breaker (MCCB), residual current circuit breaker
(RCCB), set of contactor,thermal overload relay, autotrans, condenser,
refrigerant gas , gas filter drier, drain pipe, belt drives, fan/motors
bearing,pilot light, on/off switch button, strainer thermostat switch,
thermostatic expansion valves (TXV), pressure gauge, high pressure
switch, low pressure switch and all other related works for minor repair.
iv) Consumable Materials The service contractor shall supply the
following consumable materials as and when required:- a) All oils and
grease required for lubrication of pumps, fan bearings, motor bearings,
pivot and other moving parts. b) All minor water cooled packaged unit
parts, such as belting, 230-240 VAC relay, contactor, wire, indicator,
push buttons and switches. c) All cotton wastes, soap detergent,
chemical treatment and other cleaning materials required for cleaning
purposes. The costs of consumable materials shall be included in the
fixed monthly/quarterly/half- yearly rate quoted by the service
contractor and shall not be charged separately.
v) Tools and Equipments The service contractor shall posses adequate
relevant and caliberated tools and equipment such as portable
temperature meter, surface temperature meter, relative humidity meter,
dew point meter, manifold set, air flow meter, electrical multimeter,
clamp meter, spanner set, screw driver set, players set, drills, allen key
set, gloves, and goggle. The service contractaor shall make all tools
and equipment available as and when required by the department.
vi) Respone Time The service contractor representative or technician
is required to respond immediately to the service/breakdown request to
minimize the time the system and/or equipment is not available for use.
The service contractor representative or technician shall be present
physically by authorized and competent personnel within their trades of

 competency to rectify or to investigate a fault or complaint after it was
reported by the client. The service contractor representative or
technician shall standby 24 hours to asists the department in the event
of emergency/breakdown of equipement/system.
CHILLED WATER Months 1 24
PUMP ROOM
i) Daily Schedule to start and stop ACMV system The service

service and maintenance of the ACMV system. Job scope;- - The
cooling tower and chilled water pump set are connected at the control
panel system in Blok Orkid and Blok Cempaka. The service contractor
required to ensure the ACMV system works properly.
iii) Respone Time The service contractor representative or technician
is required to respond immediately to the service/breakdown request to
minimize the time the system and/or equipment is not available for use.
The service contractor representative or technician shall be present
physically by authorized and competent personnel within their trades of
competency to rectify or to investigate a fault or complaint after it was
reported by the client. The service contractor representative or
technician shall standby 24 hours to asists the department in the event
of emergency/breakdown of equipement/system.
