INSTRUCTIONS
This report shall be completed every month and sent to technical and marine superintendent of the vessel. The report should have detailed to show
the amount of work carried out in the month. There should always be supporting pictures inserted in the report.
Ship side pictures need not be included every month; however they should be available in every alternate month report.
Pictures: The resolution of each picture should be only 96 dpi.
The following principles must be satisfied:

RESULT :
This column shall indicate an assessment of condition and is graded in the following (5) categories:
GRADE 1 = GOOD CONDITION
GRADE 2 = SATISFACTORY CONDITION
GRADE 3 = SLIGHTLY BAD CONDITION, EASY TO REMEDY
GRADE 4 = BAD CONDITION, POSSIBLE TO REMEDY DURING VOYAGE OR AT NEXT PORT BY CREW.
GRADE 5 = VERY BAD CONDITION NEEDS IMMEDIATE REPAIR BY REPAIR TEAM
REMARK :
This column is intended for any remarks related to the mentioned item.
WORK DONE :
This column is to be used for recording work carried out on the items mentioned. The following codes are to be used for indicating the nature of
the work carried out:
01 INSPECTED BY SHIP STAFF
02 INSPECTED BY CLASS SURVEYOR
03 OVERHAULED
04 PARTLY RENEWED (INSERT IN REMARKS DETAILS)
05 RENEWED
06 CLEANED/GREASED
07 DESCALED

This column is to indicate the time that the work described in the above column took place. The format to be used is: MONTH/YEAR, e.g. 01/05
APPENDIX 1:
1. Size of picture should not more than 96dpi.
How to set pictures into 96dpi
1. Select Object/Photo
2. Choose Format Tab
3. Right click on Compress Pictures
4. Apply to all pictures in document
5. Change resolution into Web Screen
6. Then choose OK
2. Each pictures must have a proper description at bottom photograph, clear and show good maintenance followed onboard
3. Old pictures cannot be added into this section.
4. All the pictures should be taken with date when picture are taken. Digital camera onboard should be set the date on so it can appear on
photograph.
