OBJECT: REQUEST FOR AN EXTENSION OF 30 DAYS FOR THE LICENSES OF TWO CAPTS.

Ceiba Intercontinental Airlines with base in Malabo (Equatorial Guinea), would like to send this
request for extension of licenses for 30 days, due to the expiration of the documentation of some
commanders for the next dates of this month of July, due to the lack of shifts available in the
simulators of the two contracted Training Centers (GTA Madrid and Ethiopian Airlines Addis
Ababa) approved by ECAA.

RISK OF ANALYSIS OF SUCH EXTENSION

It is good to know that in the operation of this company 16 sectors are carried out weekly, each
sector having a flight duration of 45 minutes, a weekly flight time that does not exceed the number
of hours required each week according to the requirements of Ethiopian Civil Aviation Authority
and taking into account the rest period of the pilots.
This Direction of Operations together with the Operational Safety Department have taken into
account the performance of the pilots on each flight, the work period, the workload, possible
mitigations as well as the risk index. In order to guarantee operational safety according to the
requirements and recommendations, a competency plan has been designed for pilots with 30-
days extended licenses, a weekly program in order to guarantee the operational safety of flights
as follows:
Under the supervision of a qualified Fight Instructor on the B737 NG:
1. Review of the theoretical classes of General Knowledge of the airplane and the different
systems that make up the B737 NG airplane. Normal and abnormal operating procedures,
SOPs and weight and balance.
2. Review Threat and error management (TEM) and human factors Skills, knowledge
and behaviors to be assessed:
a. Maintain effective lookout
b. Maintain situation awareness
c. Assess situations and make decisions.
d. Set priorities and manage tasks. Maintain effective communications and
interpersonal relationships.
e. Recognize and manage threats.
f. Recognize and manage errors. Recognize and manage undesired aircraft States.

3. For the Skill, the pilots with the license extension and class 1 medical certificate, of 30
days, will make a training flight, making 3 touchdowns and 3 takeoffs, this will be
supervised by a qualified instructor captain on said aircraft, to Increase the pilots' skills
regarding the handling and control skill of the plane.
