CHECKLIST FOR TRANSFERRING FROM BED TO WHEELCHAIR

Materials needed: Adjustable bed, Wheelchair and Gait belt

Procedure: DONE NOT DONE


1. Prepare all the necessary materials needed
2. Explain the procedure to the patient
3. Assist physical ability to move and ability to understand
instructions and willingness to participate.
4. Assess the client comfort and discomfort when moving
5. Advise patient to follow caregiver instructions carefully to
facilitate safe ambulation and transferring.
Performance:
1. Perform hand hygiene
2. Position the wheelchair 45degree angle to the side of the
bed. Lock all the wheel and fold the footrest
3. Place gait belt on the waist of the client
4. Instruct the client to grab the opposite armrest of the
wheelchair with the strong arm then support the weak leg
with your knee or tip of the feet to prevent slip off.
5. Support the body by holding the gait belt, support the weak
side of the body under the axilla toward the back with the
other arm
6. Ask the client to pivot at the count of three towards the
seat of the wheelchair
7. Pull the client towards the backrest of the wheelchair using
the gait belt and position properly
8. Unlock the wheel and push the wheel chair
9. Do it Vice versa
Evaluation:
1. Note the client muscle strength and discomfort upon
transferring.
2. Document the client reaction about the procedure
3. Carry out the completion of the procedure
