AFFIDAVIT OF SELF-ADJUDICATION

I, EMMANUEL F. GOMEZ, of legal age, Filipino, single and a resident of Bonifacio Heights
Condominiums, Building Dipolog 405, Taguig City, after having been duly sworn to in accordance
with law, depose and state:

1. That I am the only son of RAPHAEL D. GOMEZ who died without any last will and testament
on March 14, 2023 in Pasig City, as evidenced by his Death Certificate issued by the City of
Makati hereto attached as Annex “A” and made an integral part of this Affidavit;

2. That at the time his death, my father had no debts, liabilities or obligations to any persons,
agency or institution;

3. That at the time of my father’s death, he left several real and personal properties, situated in
Pasay City and more particularly described as follows, to wit:

2 A 156 sq. m. parcel of land located in Western Php2,567,000
Bicutan, Taguig City, described in detail in the

3 A Banko Sentral ng Pilipinas (BPI) bank account Php5,678,000
with account number 7685 4352 0987.
4 A condominium unit located in Bonifacio Heights Php10,897,000
Condominiums located in Fort Bonifacio, Taguig
City and described in detail in Condominium
Certificate of Title No. 768490, attached as Annex
“C”.
5 A share of stock in SM Investment Corporation Php4,567,000
(SM) with Stock Certificate of Title 764538,
attached as Annex “D”.

4. That pursuant to Rule 74, Sec. 1 of the Rules of Court, I hereby adjudicate unto myself the
above-described real estate by means of this Affidavit and hereby files the same with the Register
of Deed of Taguig City with the request that the said adjudication be made effective without judicial
proceeding as prescribed by the aforementioned provision of the Rules of Court.

IN WITNESS WHEREOF, I have hereunto set my hand this 17th Day of October 2023 in
Taguig City, Metro Manila, Philippines.

 THAT pursuant to Section 1 of Rule 74 of the Revised Rules of Court of the Philippines, and
we being all of age and with full capacity to contract, we do hereby adjudicate unto ourselves
properties described above, in equal shares.
