Instructions for using Quality of Services Data Analysis Sheet

Indicator Sheet:
- This sheet will automatically calculate the Quality of Services score for each service provider. This sheet is p

Data Sheet:
- Data should only be entered in white/active cells. All cells which have formulas have been made inactive/grey
- Column A: enter date assessment completed in dd/mm/yy format
- Column B: enter name of service provider
- Column C: enter location. For ease of analysis it is suggested that you use labels, i.e. that data is entered in
corresponds to the particular location. i.e. 201 = district 1, 202 = district 2. You will need define the specific loca
context.
- Columns D-F: select response from the drop-down list.
- Columns G-N: see suggested labels under 'multiple choice selection'. It is recommended that if the responde
a '1' is put in the relevant column(s).
- Columns O-AV: see 'indicator rating' options under suggested labels sheet. For each indicator (i.e. 1.1, 2.1, 2
number for the rating (i.e. 3=achieved). For formulas used in this sheet to be correct the suggested labels must
are protected and will calculate the average score for the standard.
- Column AW: this column is protected. It will automatically calculate the average indicator rating score of the s

Suggested Labels Sheet:
