In connection with our common goal and effort for a Drug-Free Workplace, we are respectfully and
humbly requesting for the screening/drug test schedule on behalf of E.F. Alamillo Construction and
Supply. 10 out of our 25 employees will conduct a random drug testing by your good office.

Further, we are requesting that we may be furnished with the result thereof for our record purposes
and CADAC requirement for business permit renewal.
