The participant was referred to the trainee clinical psychologist by the Addiction

Treatment Centre of Punjab Institute of Mental Health for assessment due to complaints of anger

outbursts and insomnia.

History of Present Illness

The client was first exposed to substances at the age of 14 when he tried white powder

(Opioid) with his friends at school for recreational purposes. It did not interfere much with his

occupational and family life as he could afford it and only took it with friends. He belonged to an

upper class family where taking drugs was not a big deal. At the age of 16, he had a physical

relationship with a girl. The participant used drugs to improve the quality and duration of sexual

activity. The physical relationship ended after 2 years but the client continued to use heavy

amount of opioid. The client started using 0.5 grams of opioid but later he increased his amount

to 2 grams. He also changed his method of drug intake to increase its effect. At first he used to

take drugs by smoking but later he started injecting it. It gave him the energy and made him

forget all his problems. The client reported that having money for drugs is not a problem for him

as he belongs to a rich family. His parents and siblings provide him with money which he uses

for durgs. The client attempted suicide twice. In his first attempt in 2017, he took sleeping pills

and cut his arm with a blade. His sister found him in his room. She told her family who took him

to the hospital. In his second attempt in 2019, he jumped into the canal water but he changed his

mind and came out. The client also frequently consumes alcohol with his brothers .The client has

been admitted in various private and government addiction treatment centres for more than 80

times. The longest duration the client survived without drugs is 5 months when he was admitted

in a private clinic. The client reported that he has learned some techniques that allow him to deal

with withdrawal symptoms when he is admitted in a clinic but as soon as he leaves an institute he

starts taking drugs again. The client says that he does not want to leave drugs as it gives him

pleasure and peace.


Background Information

Family History

The client's parents had been married for 45 years when his father passed away. They had

an arranged marriage and had a satisfactory relationship with each other.

The client's father was 75 years old when he died due to heart problem. He had done

intermediate and was a businessman. He was even-tempered and the client had a close

relationship with his father.

The client’s mother is 65 years old. She has done intermediate and is a landlord. She is

short-tempered and the client does not have a satisfactory relationship with her.

The client’s eldest brother is 42 years old. He has done bachelors and works at his

father’s business firm along with his other two brothers. He is even-tempered and the client has

an unsatisfactory relationship with him.

The client’s second brother is 40 years old. He has done intermediate. He is short-

tempered and the client has an satisfactory relationship with him.

The client’s third brother is 37 years old. He has done bachelors. He is short-tempered

and the client has an unsatisfactory relationship with him.

The client’s sister is 35 years old. She has done bachelors and is a housewife. She is

even-tempered and the client has a satisfactory relationship with him.


General Home Atmosphere

The client lives in a joint family system. His mother is the authoritative figure in the

house. He belonges to a high socioeconomic status. The general atmosphere of his home pleasant

pleasant but is disturbed due to the client’s drug-seeking behavior.

Past Psychiatric Illness

As per the client’s reporting, there is no psychiatric illness in the family but the client

reported that his brothers consume alcohol frequently which indicates that his brothers might

have alcohol use disorder.

Personal History

As reported by the client all developmental milestones were achieved on time and the

developmental period was smooth. As there was no informant available information regarding

the developmental period could not be asserted. The client had a happy childhood. He used to

play cricket, volleyball and took swimming classes. He had many friends in his school. After he

started taking drugs, he likes to be alone most of the time. He does not like to socialize.

Educational History

The client started going to a mainstream school at the age of 3. He did not have a good

relationship with his teachers. He used to enjoy with his friends a lot. He was an average student.

He stopped going to school in 9th grade as he did not like the environemtn at school.

Sexual History

The client achieved puberty at the age of 13. He did not have any difficulty adjusting to

the physical changes as his friends had already told him about them. He is heterosexual and has

had a physical relationship with a girl at the age of 16 which lasted about 2 year. The physical
relationship began 1 month after the relationship started. The girl cheated on the client with

someone else so he broke up with her.

Premorbid Personality

The client had an extroverted personality. He used to hang out with his friends a lot. He

was even-tempered and his mood was pleasant most of the time.

Test Administration

The test was administered in a quiet and well-lit room in 15 minutes.
