YDx Learning Club Declaration Statement.

We, the undersigned, collectively acknowledge and agree to the following terms and
conditions:
• We confirm that we have formed ourselves into a YDx Learning Club.
• We understand that we must complete and send this declaration before 16:00 on the
16th May 2024, so that we can be invited to the selection meetings with SADICO.
• We understand that the selection process will be based on us as a team but each
individual will also be assessed, including our roles in the Learning Club.
• We understand that any information shared during the group selection process will be
considered confidential and proprietary to SADICO.
• We agree not to disclose any sensitive or confidential information obtained during the
group selection process to any third party, either during or after the interview.
• We understand that any materials provided to us during the group selection process,
including but not limited to technical documents, software, and data, are SADICO’S
property and are to be used solely for the interview.
• We acknowledge that any written or verbal assessments conducted during the group
selection process are the property of SADICO and may be used for internal evaluation
purposes, kept within the Protection of Personal Information Act (POPIA).
• We understand that the YDx program is funded, with a limited budget, by the
Unemployment Insurance Fund (UIF) and SADICO will make every effort to make the
project a success, including the R2 000 month stipend per person for 6 months. In
addition though, we promise to also come to the party with whatever we can do to make
the project a success, including laptops, premises to meet, where we have resources.
• We agree to conduct ourselves professionally throughout the group selection process
and to adhere to the SADICO code of conduct and policies.
• We understand that providing false or misleading information during the group
selection process may result in the termination of our candidacy as a learning club.
• By signing below, we confirm that we have read, understood, and agree to abide by
the terms and conditions outlined in this Group Selection Declaration Statement.

NB: Commitment to the declaration (ALL to be signed completed, initialled and signed DIGITALLY). This declaration statement must be
submitted to your Cluster manager before 16:00 (4pm) on Thursday the 16th of May 2024. Thereafter, you will receive information on the
date of the selection meeting only when you have submitted your learning club declaration, only learning clubs that have signed and
submitted their declaration shall be invited to attend the selection meeting. You are not allowed to print, sign and send. ALL SIGNATURES
MUST BE DIGITAL. The YDx-APP-TBD to be used for the members who do not have Application codes. Just a reminder that each learning club
has 12- 16 members, you are not allowed to have members below 12 members or more than 16 members.
