Reminders for Face-to-Face Final Examination

For BSN Level II, III and Level IV Students

Period: June 13-18,2022

1. Wear white CON Shirt/ white polo shirt and non-fitted maong pants with white laboratory gown for
BSN Level I Students.

▪ Wear white clinical uniform without apron for BSN Level II, III and IV students.
2. Wear white KN95 facemask
3. Bring individual hygienic kits for your personal use

▪ Alcohol
▪ Liquid hand soap (pump/ Squeeze bottle)
▪ Tissue wet and dry
▪ Face Towel

4. Bring photocopy of Vaccination Cards

5. Photocopy of Phil health insurance card (PHIC) or any health insurance
6. Waiver form signed by guardian or parents.
7. Bring black ballpen and pencil (Mongol no. 2) that will be used during the Final exam.
8. Minimum amount will be collected for test paper fee.
9. Bring food and personal necessities – snacks, drinking water, juice and candies
10. Bring garbage bag per group.

Note:

▪ Health Declaration Form available at the CvSU gate (to be accomplished before entry and to submit
at the guard on duty).
▪ All students are required to fill up the CvSU health monitoring sheet and to be submitted to their
respective instructor/adviser.
▪ Follow the schedule as advised by your instructors respectively.
