Re: CERTIFICATE OF NON-COVERAGE UNDER DOLE 174-17
Dear Madam:
I had a conversation with Madam RAIN of DOLE (Contact Details: 8367-3180/8283-5044) a day ago.
We were seeking to have a DOLE 174 Certificate of Exemption but her division only caters to Dole
Registration, and as such we were informed the we are already Registered since year 2016. I was
referred for assistance & to make a Request Letter to Sir ADRIAN DUMAGUIT (of DOLE
REGIONAL OFFICE via Email: etr.cfo@gmail.com). Mr. Adrian Dumaguit eventually replied to make
my request be directed instead to your good office together with the List of Requirements.
Our small company, GIRON MACHINE SHOP & METAL WORKS (which is a single proprietorship
with 8 personnels) works for Some Animal Feeds & Some Human Food Manufacturing Companies
doing FABRICATION and/or REPAIR WORKS of their SMALL MECHANICAL PARTS/MACHINES
to suit the variety of their mechanical needs & usages. As such, we were classified as a Service
Provider. Maybe around a couple of years ago we were instructed to issue Billing Statement
instead of Sales Invoice to compliment being a Service Provider.
As a service provider of this Very Distinct Nature, we were required to acquire & present them a
CERTIFICATE OF NON-COVERAGE UNDER DOLE 174-17 for re-accreditation purposes. We don’t
cater to provide manpower services for maintenance nor operation of the plant.
It is in this connection we are seeking your good office’s assistance regarding the acquisition of this
Exemption Certificate that our company needs so as to be able to get work again from their
Company.
