Subject: Request for transfer of CGHS Card to Kolkata upon transfer

from Guwahati to Kolkata – Reg.

Kind attention is invited that, vide transfer order dated 28.02.2024 in file
no. Est-19/1/2024-EST-HO issued by Directorate of Enforcement, HQ, New
Delhi, the undersigned was transferred from Hqrs, New Delhi and posted to
Kolkata (Copy attached).

It may kindly be noted that the following CGHS cards had been allotted
to me while I had been working at Guwahati.

Health & Family Welfare, Department of Health & Family Welfare.

It is certified that this officer and his family are residing in Kolkata in a
CGHS covered area.
