1.0 PURPOSE:
1.1 To provide guidelines for safe medication management and administration to
patients.
2.0 SCOPE:

4.0 POLICY & PROCEDURE OF ADMINISTRATION OF MEDICATION


4.1 POLICY & PROCEDURE OF PROCUREMENT OF MEDICATION:
Initially JUPITER SCAN CENTRE dose not store bulk amount of medicine. Only few
emergency & high risk lifesaving drugs are there in crash cart trolley & emergency
medicine tray, which have procured locally from outside pharmacy under professional
supervision of Medical Director & Nursing.
1. In JUPITER SCAN CENTRE we don’t administer any medicines to patient.
2. In case of any emergency, if any administration of mediation is
required then it is documented in the medication card.

3. All the entries in the card shall include the:


Date of received.

Name of medication

Dosage

Route of administration

Timing

Name and signature of the person who has administered the

medication.
5.0 POLICY ON ADVERSE DRUG REACTION:
 In JUPITER SCAN CENTRE we don’t administer any medicines to patient, still in case of any
emergency if any medication is administered to any patient and he develops any unusual
incident like hallucinations, shortness of breath, rashes, itching etc OPD nursing staff shall
immediately report to the consultant.
 The patient will be transferred to the nearest hospital immediately for further treatment.

 Doctor should fill the Adverse Drug Event / Reaction Form. The form to be submitted to
Quality Department. Corrective and/ or preventive actions are taken based on the
recommendation
6.0 POLICY ON STORAGE OF DRUGS:
Medicines are stored in cool temperature & under room atmosphere, moisture free area.
All medicines should be sealed, tamper proof, leakage proof condition. Medicines stored
in Crash Cart trolley & in emergency tray should have a proper checklist with quantity,
which checked daily. Medicine storage should be under supervision of Nursing in charge.
6.0 POLICY ON PRESCRIPTION OF DRUGS:
Prescriptions are written in legible manner with date, time and full signature of the doctor
including the registration number of the doctor.
7.0 POLICY ON DISPENSING OF MEDICINE:
 Dispensing of medication shall be conformed to applicable laws and regulations
governing pharmacy practice in state of west Bengal and India.
 The order shall be screened for appropriateness of drug, dose, and frequency, route of
administration, therapeutic duplications, drug-drug interactions, allergies, and
formulary status. Labeling requirements (at a minimum, labels must include the drug
name, strength; frequency of administration (in a language the patient understands)
shall be documented and implemented by the organization.
 Use of physician’s samples in the clinic is not permitted.
 Expiry dates shall be checked prior to dispensing.

 In case of contaminations, short expiry drugs, expired drugs etc., drugs shall be recalled as
per the procedure. Drugs to be sent back to pharmacy and informed about this status in
written to In-charge.

 All medications shall be verified by the Departmental Head at the time of receipt of
goods and the same shall be checked for damages and contaminations.

 According to the feedback from patients or staff, the pharmacy should have a recall
procedure if the medicine is contaminated or expired.

 In case of any such incidence, same shall be returned back to the pharmacy along
with the bill (batch and serial number).

8.0 POLICY ON DISCARDING OF MEDICINE:

 Expired medicines will be handed over to vendor, & received the new stock against the
same.
 After medication administration, the ampoules will be discarded in Blue marked PPC
container.
 All the used strips are marked as open date.
 Used Contrasts bottles are stored till old lots are finished & new lot received. After
receiving new lot, old plastic bottles are discarded In Red BMW bin, & Glass bottles in
White PPC Container.
 Medication administration & discarding are done only by registered medicine
practitioner & by staff nurse/brother.
