1.0 GENERAL

The work permit procedure is a common device to ensure that communication takes
place and hazards are controlled.

Restricted Areas are potentially hazardous. However, knowledgeable people using


proper procedures can perform efficiently and safely. The work permit procedure is the
most important procedure used to ensure safe working condition for large numbers of
people. It must be followed.

2.0 DEFINITION

Restricted Areas are those areas that require work permits. Those areas include oil
handling facilities, pipelines, power lines and radioactive and explosive handling areas.

Two people are involved in issuing and receiving work permits. The issuer is referred to
as the “Safety Department” and the receiver is referred to as the “Authorized
Craftsman”

3.0 SCOPE

A Permit to Work is required for:


 Installation containing hazardous or flammable materials.
 Excavation or trenching work.
 Confined Space.
 Electrical installation or work on electrical apparatus
 Hot work.
 Working at height
 Other hazardous work
 Lifting activities
 Non-routine activities on site

4.0 FUNCTIONS

The Work Permit authorizes specific construction, maintenance, inspection or repair


work to be conducted in the company’s restricted areas. The permit serves as an official
written record of conditions and requirements agreed upon by the issuer and receiver. It
lists minimum safety precautions to be taken and hazards that must be controlled.

 Release of Flammable Liquid or Gases


This form is required when opening hydrocarbon lines that may release
flammable or toxic materials

 Working at Height
This form is required when working at height more than 10meters away from the
ground. The frequency of the form is weekly.

 Excavation and Trenching Work


This form is required when have a excavation and trenching works more than
3meters depth.

 Lifting
This form is required when any lifting at constructions including using tower
crane, mobile crane, crawler crane or any lifting machinery.

 Hot Work
This form is required when using sparks or flame producing equipment and for
vehicle entry into a restricted area.

 Cold Work, Vessel Or Confined Space Entry


This form is required for general work such as sand removal, brush painting,
insulating, cleaning, etc.

 Non Routine Activities


This form is required for any non-routine activities that will be identified by
Project Safety and Health Officer.

All work in Restricted Areas must have some type of Permit. Work must be
performed according to the instructions and precautions on the permit

5.0 RESPONSIBILITIES

It is the responsibilities of:

5.1 Safety Officer


 To inspect the permit issued to the holder at the work side.
 To review on the Permit to Work system when necessary and report it to the
Safety and Health Committee.

5.2 Permit Issuer


 To ensure on the execution of the Permit to Work.

5.3 Permit Holder


 To adhere to the Permit to Work issued.

6.0 ISSUANCE AND APPROVAL

The craftsman must request a Work Permit from a certified issuer (Safety Department)
before doing any work in a Restricted Area.

The issuer will grant the work permit after he has visited the site with the craftsman
responsible and reviewed the hazards applicable to that particular job. If the work con
template involves any change, addition, or deletion in the facility, an engineering review
and authorization are necessary.

7.0 PRECAUTIONS

Each permit contains a checklist of precautions against common hazards. Such a list
cannot include precautions against all hazards.

It is the duty of both the issuer and the receiver to review the job, anticipate what
hazard might arise, and see that proper precautions have been specified on the Permit
before it is signed.

One specific precaution applying to all work is to clear the area of people not required
for the job so as to avoid their being exposed to unnecessary hazards.

If people enter an area where they could be exposed to undue danger, the work should
be stopped until the people are cleared from the area.

If conditions become unsafe during the course or work, the operation supervisor may
stop the work and cancel the permit.

The authorized craftsman has the responsibility to stop the work and advise the
operation supervisor any time he feels the safety of the job does not meet the
conditions of the work permit.

8.0 HANDLING OF ISSUED WORK PERMIT

A work permit is valid for only one shift but may be extended for one additional shift
with proper approval. Exceptions in excess of 24 hours may be granted in special cases,
provided certain precautions are taken.

The receiver of the work permit must keep the permit on the job site at all times.

9.0 CLOSING OUT AND FILING THE PERMIT

Either the issuer or the receiver can revoke a Work Permit. This must be done if a
hazardous situation develops after work starts or if there is any other change in
circumstances likely to effect safe working conditions.

When the job is completed and at the end of each day’s work, both issuer and receiver
must close out each work permit. The work permit will be filed and kept by the
operation supervisor for three (3) months.

10.0 PROCEDURES

10.1 Appointed Permit Issuer:


 Project Manager
 Sub-Contractor
 Site Supervisor
 Section Engineers / Executives

10.2 Approving Authority:


 Safety Officer
 Site Safety Supervisor

10.3 Work without permit is prohibited. Action such as penalty shall be taken if work
involving sub-contractors do not have the Permit to Work.

10.4 The Permit Issuer shall review the scope of the job with the Permit Holder. If site
inspection is necessary the Permit Issuer will accompany the Permit Holder for a
joint inspection, followed by a safety briefing session.

10.5 Upon completion of safety briefing, the Permit Issuer shall fill in the Permit to
Work form. The condition on the form shall be explained and agreed upon by the
Permit Holder.

10.6 The Permit Issuer shall inform and obtain the approval from the Project Manager
whose area will be directly affected by the work before issuing the Permit to
Work.

10.7 If the Permit Holder fails to comply with the conditions specified in the Permit to
Work form, the Permit Issuer or the Project Manager shall withdraw the Permit
to Work and report it to Safety Officer.

10.8 The Permit Holder shall have in his possession the Permit to Work and show it to
the relevant party upon request.

10.9 If the job could not be completed within the agreed period, the Permit Issuers
may seek for the extension of the Permit to Work and shall provide a progress
report for justification.
10.10 Upon completion of the job, the Permit Holder shall notify the Permit Issuer.

10.11 Both parties shall then inspect the workplace to ensure there are no unsafe
conditions after completion of the job.

10.12 The Permit to Work shall then be closed out with the signature of Permit Issuer,
Approving Authority and Permit Holder.

10.13 The Permit to Work shall then be filed and kept in Safety Department for the
period of six (6) months
