(Certification of Grades) with corresponding units indicated with
school seal (2nd Sem, SY 2021-2022) not less than 18 units. School
must indicated the exact Equivalent grade of numerical rating
issued by the Registrar (ie. 1.5-90.00%)

B. Original or Authenticated Certification of Academic Load Enrolled

issued by the School Registrar (1st Sem, SY 2022-2023) not less
than 18 units

C. Latest Military Science Grade duly signed by the Chief Clerk and
Commandant or grades from the school registrar (2 nd Sem, SY
2021-2022)

D. Equivalent Points for the Aptitude, Bearing and Service

E. ROTC Cash Incentives Performance Data Sheet

F. Computation for the Grade Point Average

G. Bio-data with Two (2) copies of 2x2 ID Pictures

H. Duly Accomplished Engagement to Serve

I. Security Declaration

J. ROTC Cadet Cash Incentives Contract/Undertaking duly signed by

the applicant and notarized in a Notary Public

K. Birth Certificate from NSO

L. Physical Fitness Test Result
