This is to authorize my son, Al T. Kimmayong, identification number B07-18-000907 to

make a request for, process, apply for, and claim the Certified True Copy of the Tax
Declaration of our property covered by TCT No. P-9476 under my name. Due to reason
for personal commitments, I am unable to be present to perform this task personally.
Therefore, I have granted my son, Al T. Kimmayong full authorization to act on my
behalf. This includes the signing of documents, receipt of the item or document, and any
other actions necessary for the claim.

Attached to this letter, you will find a photocopy of my identification National ID for
verification purposes. Additionally, my son will present his driver’s license identification
ID upon request.
