Family Assessment Tool
A systematic way of understanding the family
and to aid them in evaluating the impact of
illness on a person and on his/her role in the
family.
Can be used to gather data about the family’s
systems, functioning, culture and experiences.
Information about the way the family functions
in nurturing its members, problem solving and
potentially more effective for management of
child’s health care.
They enable the nurse to work more effectively,
such as collaborating with the family in planning
for health maintenance and health promotion.

1. STRUCTURE
GENOGRAM

- It is a diagram which used in illustrating a


person’s family members such as how they
are related and their medical history.

- It described as a graphic representation of


families that charts the interactional
processes over three generations
(McGoldrick, Gerson, and Shellenberger1999)
Components of a complete genogram
1. Names, ages of all family members
2. Exact dates of birth, marriage, separation, divorce,
death and other significant life events
3. Information covering 3 or more generations
4. Illness (hereditary and significant illnesses)
5. Firstborn of each family to the left and other siblings
sequentially to the right
6. Indication of which members live together in the
same household
7. Names of 2 families with the address of the index
family
8. The informant/s
9. Date the genogram was generated
