8 PAGES TOTAL – 1 MAIL PACKET with these documents in this order:

1. COVER LETTER
2. ORDER FOR DEPOSIT, MANAGEMENT & INVESTMENT
3. PRIVATE BOND FOR INVESTMENT
4. CERTIFICATE OF LIVE BIRTH with “Accepted For Value” STAMP
AND 5 lines filled out/signature
5. FORM 56 FOR PUERTO RICO (Melba Acosta appointment NOT Jacob Lew)
6. FORM 56 BACK (Melba Acosta appointment NOT Jacob Lew)
7. WBEN FRONT
8. WBEN/AFFIDVAIT BACK

ORIGINAL WET INK VERSIONS of all documents, make sure to make

good quality, color copy versions saved in your files for backup. Email to
us the final versions that you are about to mail in this order so that we
can check it and also save a copy as backup (in case your computer
crashes or something).

For the CERTIFICATE OF LIVE BIRTH, it’s good to get a Certified Copy
with a Raised Seal, much more preferred, or if not possible, a copy of
the Certificate is OK, just make sure it has the A4V Stamp and your
original signatures when it goes out.
Put all docs in the above order with no staple or paperclip, in a large
paper-sized envelope so there is NOT ANY FOLDING ( ex 9” x 12”). Fill
out the to and from on the outside of the envelope with the SECURED
PARTY CREDITOR’s address. Fill out the green card with either the
SECURED PARTY’S ADDRESS – or – THE DEBTOR/U.S. CITIZEN’s address,
so that you get it back easily and soon.

MAKE SURE that you Get a Certified Mail Number/slip before mailing

And to put the rip-off slip at the top onto the Green Card to place in
box 2., “Article Number” so you have proof of delivery.
