We, the researchers from Mapua Malayan Colleges Mindanao of Grade 11-Bell,
are currently conducting a study entitled, Untold stories of cop’s children: A
Phenomenology. This study aims to better understand the lived experiences of
children whose parents work as a Philippine National Police Personnel.

In line with this, we have chosen you to be one of our respondents. Thus, we are
asking for your cooperation and effort to answer all the questions honestly. Your
support is very important to us, and it will be helpful to the completion and success
of the study.

I understand that my student/s have been asked to participate in a study entitled:

Untold stories of cop’s children: A Phenomenology.

He/she will be asked to undergo guided interview session which will take about 30
minutes to 1 hour. I understand that he/she does not have to participate.

If he/she does participate, he/she can quit at any time. I also understand that
