CONTRACT OF SERVICES

KNOW ALL MEN BY THESE PRESENTS:

This contract into by and among the Municipality of Bacarra herein represented by: HON.
Nicomedez Dela Cruz Jr., Municipality Mayor and Chairman, municipal nutrition Committee (MNC),
hereinafter after referred to as MNC; the Barangay Council of 19-A Tambidao herein represented by
Hon. Antonio A. Visitacion Barangay Captain and Chairman, Barangay Nutrition Committee (BNC),
hereinafter referred to as BNC and Mayrose D. Damo of legal age, Filipino, and a resident of Barangay
19-A Tambidao hereinafter referred to as SCHOLAR.

WITHNESSETH:

1. That for and in consideration for the following:

a. Nutrition training materials and corresponding office supplies.

b. Allowance _______ for every month of actual services with the program hereinafter mentioned
to be provided thru the Municipal Mayor upon submission of his/her monthly report.
c. Honorarium/incentive of Php 1,500.00 for every month of services with the program hereafter
mentioned to be provided by the Barangay Council through the barangay Captain upon
submission of his/her monthly report.

Which consideration shall be provided by the MNC to the SCHOLAR and for which consideration
the scholar has agreed to work with the MNC on the Philippine Plan of Action for Nutrition
(PPAN) in particular under the Barangay Nutrition Scholar Program (BNSP).

2. That the SCHOLAR shall first undergo training to be designed by the PNC/MNC and to be
conducted before the actual one (1) year within the designated barangay.

3. That the SCHOLAR will work under the Barangay Nutrition Committee including but not limited
to the performance of the following basic tasks:

a. Problem identification-identify the nutritional problem of the families and the community
as a whole by generating the data on:
1. Family Profile (Situational Analysis)
2. Electronic Operation Timbang Plus (eOPT Plus)
3. Spot Map (updated and labeled accordingly)
4. Problem tree/Causal Model of Malnutrition

b. Social Preparation
1. Presentation of the identified problems of the barangay people
2. Solicitation of action/recommendations from community on how they will jointly solve the
identified nutritional problem
3. Organization of the community into barangay networks
4. Creation of an advisory committee which could be the Barangay Nutrition Committee
(BNC)
5. Preparation with the community of a Barangay Nutrition Action Plan (BNAP)

c. Linkage building-Referral system

d. Action Planning: BNS Action Plan and Barangay Nutrition Action Plan (BNSAP and
BNAP)

e. Delivery of Services
1. Gives nutrition information and Education materials targets families
2. Promote utilization of iodized salt and other fortified foods to families
3. Conduct supplementary feeding whenever necessary
4. Provide seeds and seedlings for backyard/vegetable gardening.

f. Monitoring and reporting

1. Monthly weighing of 0-23 months including new-born babies during the period as well as
Underwweight,Severely Underweight, Stunted, Severely Stuted, Wasted,Severely wasted,
Overweight, Obese cases.
2. Quarterly weighing of 24-59 months PS including New Cases.
3. BNS MONTHLY ACCOMPLISHMENT Report to include other activities not listed in the
format.
4. Assist mothers in the filling up of Growth Monitoring Charts of children 0-59 months.

g. Others
1. Preparation and accomplishment of BNS diary
2. Regular attendance to BNC/BNS meetings and keeping minutes.
3. Initiate conduct of fund raising activities.
4. Assist newly recruited and trained BNS in the preparation of required documents.
5. Regular calibration of weighing scales and maintenance of its cleanliness.
6. Regular checking for calculating of age in months and determination of nutritional status.
7. Initiate the organization of community associations particularly those income generating
projects with the next in line beneficiaries has a roll over scheme.
8. Attends meetings of the BNC to explain his/her role as a BNS and the report his/her target
activities in the barangay related to nutrition.

4.That the Scholar shall recruit and train volunteers from the area of assignment who may
act as substitutes in case of voluntary resignation or any other situation.

5. That incase of voluntary resignation or any other cause of termination of services, the
Scholar shall automatically lose the insurance benefits and shall surrender to the NAO all
nutrition information and education materials and equipments supplied by the MNC and the
same shall be given to the next designated SCHOLAR.

6. That this contract of services shall take effect from January 2023 to December 2023
renewable on option of SCHOLAR upon satisfactorily upon prior advice to the SCHOLAR.
IN WITNESS WHEREOF, the parties have signed this contract this 2nd day of January 2023 at
Barangay 19-A Tambidao, Bacarra Ilocos Norte.
