Scotland, United Kingdom, a holder of UK Passport with No. 12345, hereby appoint and
constitute PEDRO DELA CRUZ, likewise of legal age, Filipino citizen, and resident of Dapdap,
Mabalacat City, Pampanga, to be my true and lawful attorney-in-fact for the purpose herein
stated:

1. To process and apply for the extension of my Philippine tourist visa with such authority
as may be necessary or appropriate to represent me in all matters concerning the said visa
extension, including but not limited to:

 Filing of necessary application forms and documents with the appropriate government
agencies or embassies.
 Providing and receiving documents related to the visa extension process.
 Making inquiries, signing, and submitting all necessary paperwork, including but not
limited to affidavits, declarations, and consent forms.
 Appearing on my behalf before relevant authorities and submitting to any required
interviews or questioning.
 Taking any and all actions necessary or appropriate to expedite the processing of the
visa extension.

I hereby give my said attorney-in-fact full power and authority to do and perform all and
every act and thing whatsoever requisite and necessary to be done in and about the premises, as
fully to all intents and purposes as I might or could do if personally present, with full power of
substitution and revocation, hereby ratifying and confirming all that my said attorney-in-fact or
her substitute shall lawfully do or cause to be done by virtue of these presents.
