SUBJECT : Spot Report on Fire Incident at Ron wood

Hardware

DATE : June 30 2017

1. Reference: Fire Incident transpired on June 30, 2017 at Ron wood Hardware
Building, Montilla Blvd., Butuan City.

2. OOA 10:00 in the evening of July 30, 2017.FO2 Gelpar A. Salga, received a
call from one Mrs. Stephanie H. Radaza, 40 years old. Informing that there was a fire
incident transpired at Ron wood Hardware Building located at Montilla Blvd, Butuan
City. Immediately two (2) unit of fire truck responded the said building. Upon the arrival
of the responding unit at around 10:05 in the evening , they perceived that the fire is on
its free burning phase, which produces heavy flames and they also perceived that the
color of the flame is black and produced black smoke for it is a CLASS A fire and had
assorted combustible materials such as rubbers. The fire was under control at around
11:00 in the evening.

3. According to the preliminary investigation conducted to the witnesses that the

fire started at the office of the Manager. Cost of damage is estimated to be around Php
500,000 pesos.

4. Evidence recovered during the fire scene investigation is as follows:

a. Burned bottle with an odor of gasoline

b. Burned lighter
