Ms. Patterson presented on 25/10/2019 visibly upset with intense fear. She claims her brain is
broken and her disease is back, due to her hearing noises like before. She is distaught with the
unablity to calm down. Considering the auditory hallucinations it is suspected to be
decompensation of schizophrenia. Consequantly diazepam 20mg was given.
Ms. Patterson has medical history of schizophrenia for 4 years, which was managed at your
hospital.
Ms. Patterson first presented on a regular check-up on 12/10/2019, stated that she has no
negative thoughts as she tends to ignore them. On examination she was well kept, relaxed and
not distressed. She was advised to continue Risperidal 2mg twice a day. On 23/10/2019, Ms.
Patterson revisted with complaints having no motivation with flattened affect and reduced
willingness to perform daily activites or go out, as well as, feeling of something wrong with
her body. She was referred to outpatient psychiatrist service at Knoclbracken hospital on
30/10/2019, and instructed to continue Risperidal.
Given the above, Ms Patterson will be transported via ambulance for hospitalization and will
require your specialized management.
