2 CASE STUDY – HOSPITAL DATABASE SYSTEM

City-A-1 is a self-contained town in Perak and has a private hospital which provides a comprehensive
medical care for its inhabitants. The hospital consists of five wards namely Reed, Pasteur, Lister, Harvey
and Holmes. Each ward has its details such as location, person-in-charge, the number of beds, last
painted, opened date and daily charge.

When a patient is admitted to the hospital, the details of the patient such as NRIC, surname, last name,
address, gender and next of kin are recorded. The patient’s admission details such as admission date,
expected operation, staff who admitted the patient, the ward id and discharge date are kept in the
database. The patient will then stay in a ward for observation and to undergo an expected operation.
When the admitted patient is under observation, the observation date and time and the type of
observation, the observation value and the staff who is in charge of the observation are recorded in the
patient observation file.

When the patient has undergone an operation, details such as the actual operation and the date of the
operation, the surgeon name and anaesthetist name who are involved in the operation need to be
recorded. Since the expected operation may be different from the actual operation, the actual operation
is also needed to be recorded for accuracy and billing purposes. Each operation type has an operation
id, operation name, operation description, theatre fee and days needed to recuperate.

When a staff joins the hospital, employment date together with all the patient’s attributes are kept. Note
that a staff can also be a patient in the hospital. (Hint: you are expected not to repeat the same attributes
in both entities)
