Given the two pieces of evidence, what are the key clinical questions to consider when caring
for this patient?

With the evidence provided herein, the first question I would ask as a caregiver would be in
regards to catheter use and whether she had been using one in her previous healthcare institution.
Although she is urine incontinent, evidence suggests that the use of catheters should be avoided.
The question would enable me to check for any complications if she had been using a catheter
previously. The second question would be about the patient’s pain and comfort. As it stands, the
patient risks developing pressure ulcers, thus I would want to ask the patient if she would prefer
a higher-specification mattress.

2. If the evidence of meaning existed, what questions would you ask about the meaning of a
patient’s experience?

In this case, I would ask the patient about how much information she has about her condition.
Having patients being informed about their condition is effective for developing patient
engagement and involvement in their care (Hopp & Rittenmeyer, 2020). Secondly, I would ask
about the patient’s experience with pain. Currently, she is in pain from the cancer and the fall
that she suffered. Therefore I would ask about her level of pain and how she has been coping
with it in order to find solutions that work best for her.

3. What are the potential conflicts between the decision that the evidence would point to and
what the patient may desire?

A potential conflict that would occur in this case would be the patient refusing a higher-
specification foam mattress possibly because of higher costs. This refusal could expose her to the
risk of pressure ulcers. If the patient had been using a catheter previously, she may reject the
decision to use other methods to aid the incontinence, thus exposing her to complications.

4. How could you use the principles of patient-centered care to resolve any conflict between the
evidence and patient desires?

One of the principles of patient-centered care is to foster patient education. In this case, I would
educate the patient about the benefits of using the higher-specification foam mattress given her
risk for pressure ulcers. I would also educate her about the benefits and risks of using a catheter,
to enable her make informed decisions.

The role of patient-centered care and patient preferences is to ensure that caregivers understand
the patient’s preferences and design care plans that align with those preferences.

The nurse should also explain the rationale for the use of a higher-specification foam mattress
and the potential benefits and risks of the use of a urinary catheter. The nurse should also explain
the options for comfort care at the end of life. The nurse should work with the patient and the
interdisciplinary team to ensure that the care plan is individualized and meets the patient's needs.
