A RESOLUTION TO OPEN SLP ASSOCIATION PASSBOOK SAVINGS ACCOUNT

AT MAKILALA MULTI- PURPOSE COOPERATIVE BANK

WHEREAS, the SLPA had formalized the change of officers, therefore the KIMSPA SLP
Association designated New President, Treasurer and identified SLPA officer who will to go to
Makilala Multi- Purpose Cooperative, Antipas, Province of Cotabato.

WHEREAS the membership fee which includes pre-operating expense shall be used as deposit
for the opening of account of the SLPA. The amount needed for the opening of savings account
shall be equally paid by each member of the SLPA;

WHEREAS, the change signatories shall be the President, Treasurer, and the Identified SLPA
officer may jointly withdraw the grants.

WHEREAS every bank withdrawal must be supported by a resolution singed by majority of the
SLPA members.

WHEREAS the members of the association have determined it to be in the best interest of the
association to open an account in Makilala Multi- Purpose Cooperative, Antipas Branch.

RESOLVED, that the association will open an account in Makilala Multi- Purpose Cooperative
Bank at Poblacion, Antipas, North Cotabato, Philippines.

FURTHER RESOLVED, that the following be and are hereby authorized to sign, execute, and
submit all documents in connection to opening of bank account in the said branch of Makilala
Multi- Purpose Cooperative Bank including the verification of signatures of authorized signatories
as may be required by the Bank.

FURTHER RESOLVED, that the above representatives are hereby authorized to transact
business, including but not limited to, the maintenance of savings, checking and other accounts,
as well as borrowing by the association and other account-related transactions deemed necessary
for the representatives to perform in behalf of the association.
