PART B: EDUCATIONAL INFORMATION AND ENTRY REQUIREMENT’S.

Applicant must complete grade twelve, matriculate or upgrade within the last three years; year 2021
and upwards. Must score A, B &C grade in English, Mathematics, Biology and Chemistry. Should have
a GPA of 2.5 and above.

PART C: DOCUMENTS/INFORMATION REQUIRED

Please attach the following documents with this application form:

1. Copy of grade 10 & 12 or matriculation certificates and transcript

2. Current reference letter from the secondary school or matriculation Centre

3. Birth certificate

4. Current character reference from a prominent church leader, preferably senior Pastor on a
proper letter head

5. Police clearance report

6. Medical certificate from a MEDICAL DOCTOR and it must be stamped

7. Attach a current pass port size photo

8. Pay a non-refundable application fee of K100.00 into the East Sepik College of Nursing BSP
cheque account No: 7008600749, Wewak branch. Attach the deposit butt with the application
form.

9. Copy of your NID card (optional)

Please Note:
❖ Applications without any of these documents or incomplete application will not be
processed
❖ Only the shortlisted applicant who met the requirements will be notified via phone to
arrange for pre-entry test
❖ Applicants who passed the pre-entry test will be further screened by the Academic
Assessment Committee for final selection
❖ All completed application forms should be submitted at the Admin front counter
❖ Application will be closed on the 31st of July, 2024.
❖ We will not accept any application documents through email or Whatsapp, please
forward all application through DHL, TNT, Post Office or Hand deliver to the admin
counter.
