Assessment Task 2: CHCAGE001 Facilitate the

empowerment of older people


Task Description: This work placement task requires the student to demonstrate their ability to use verbal and
nonverbal communication strategies to develop a positive and respectful relationship with older clients. In this
task you are required to interview one (1) client/resident with additional support needs. You can also involve the
family member or carer if they are present at the time of an interview.

Conditions needed to complete your work placement task:

 Home modification to eliminate hazards, prevent accidents such as falls

What are the goals that the client wants to meet?


 Able to maintain quality of life through proper management of symptoms of his illnesses
 Have the freedom to practice his faith and culture
 Learn techniques to reduce agitation and promote relaxation to cope with dementia, better
grasp of language and speech, improved memory and a more positive outlook on life
 Stay active to help slow down dementia
 Be free from injury through home modification and using assistive aid

List four (4) services or strategies implemented to help the client achieve the goals

1. It is ensured that client has access to medical and mental health care, that there is proper
management of medication,
2. Client is supported in meeting his religious obligations like ensuring he could hold worships
at home and in the synagogue, observe Jewish practices and participate in community
celebrations.
3. There are therapy programs like cognitive rehabilitation such as learning how to use a
mobile phone, and cognitive stimulation, that is taking part in group activities and exercises
designed to help improve memory, problem–solving skills and language ability
4. Home modification- remove items that create clutter and noise such as extra TVs or radios,
and hide things that can be dangerous such as knives

Evaluate the effectiveness of the current strategies/services in place (what works/is not working for the
clients)

 The client has not experienced any injury in his home after the modification.
 Symptoms are properly managed through ensuring his medications are taken properly and
on time.
 Enjoys the freedom to practice his culture which makes him feel valued and empowered
 There is change in behavior, will explore a new therapy to support his condition as it
progresses

What are the challenges faced by client/families/carer in terms of exercising their rights?

 Discrimination that can lead to social isolation, loneliness and poor mental health
 Barriers to accessing government facilities
 Lack of assistance

What are the strategies implemented to support the client to make his/her own decisions? Use 50 – 100
words to explain
Client is made to define his needs, goals and aspirations. In the design of his care plan, he is
consulted on every aspect of his care. Prior to any process or intervention, he is provided with
relevant information so he can make an informed choice. Explaining information is in a way that is
easiest for him to understand. He is not forced to make an instant decision rather he is given
plenty of time to think it over.

What communication strategies did you use when interviewing the client? Remember to include verbal and
non-verbal communication examples in your answer. List at least three (3) strategies for each type.
Verbal communication strategies:
 Asking for clarifications
 Speaking clearly and concisely
 Using open-ended questions to gain insights

Non-verbal communication strategies:


 Maintain comfortable eye contact
 Maintaining an open body position like not crossing the arms over the body as it may
appear defensive
 Using larger prints as client has vision problem
