Describe the information that you provided the client about the case management process
I explained to Nancy that information gathered in the case management process would be kept
confidential in line with privacy and confidentiality laws.
I informed her of her rights to complain and appeal a decision and that her consent would be
needed prior to the process.

Describe three goals that you worked with the clients to set as part of the case management
process

 Client wanted to effectively manage the symptoms of her illnesses so she could go on with
her everyday life. She also wanted home modifications to prevent her my getting injuries at
her own home.
 Client wanted to learn new skills to divert her attention from her sufferings
 To live a meaningful life, she wanted to be deeply connected with her community and gain
more friends

Describe the strengths and abilities that you have identified in collaboration with the client.
 I found her to be resilient, passionate and optimistic.
 She loves to be involved in volunteer works despite her old age.
 She is good at cooking.
 She is creative, can generate ideas and alternatives.

Describe the clients immediate, short term and long-term needs


Immediate: health checks/treatment to manage the symptoms of her illnesses, home modification,
hearing aid

Short term- counseling, learning new skills such as painting to keep her busy and happy

Long term- social activities to get her in touch with local community
Describe any high-risk situation you have identified concerning the client’s circumstances.

How did you integrate cultural considerations when planning case management with this client?

I asked client about her culture and she mentioned that physical touch is considered inappropriate
in their culture. I made sure that workers and relevant people were aware of this, and this cultural
consideration was properly integrated into her care plan.

How did you monitor the progress of the case plan


 I regularly asked client about her observation, opinion.
 I also consulted other relevant people about how client met her goals.
 I compared the outcome against our target goals.

Did you need to make any changes to the case plan and what were these changes and why?
 Psychotherapy did little to address her depression so medication was incorporated into the
treatment program. She was also made to practice self-help like doing regular exercise and
getting enough sleep.

It was discussed to him that his information would be handled in line with confidentiality laws and
that other people close to him might also need to provide important details about him.
I informed him that he could make a complaint if something was unfair, that he could appeal a
decision and that his consent would be sought prior to the process.

 Client was in a lot of pain and discomfort that he wanted to have his symptoms properly
managed. He also wanted assistance with some activities of daily living.

 Client is hopeful that he may still enjoy a productive life ahead.

 He has good time management skills, making productive use of his time.
