Bhd. (‘Amway")'s Bodykey Health Warrior 2024 Programme. I hereby consent to the terms and conditions of the Programme and acknowledge that my participation in the Programme is on a voluntary basis without any monetary payment whatsoever. | understand that my failure to comply with any of the terms and conditions may result in my immediate disqualification and/or elimination from the Programme at Amway's sole discretion. WAIVER AND RELEASE I consent to the use of my credentials, images, photos and/or videos by Amway, for offline and “online advertising and promotional purposes, except where prohibited by law. In addition, | waive s,images, photos and/or andrelease the copyrightand other rights inrespectof the said creder videos which shall belong to Amway and shall not claim any monetary payment in relation to such use by Amway. CONFIDENTIALITY | will not use or disclose to any party, any information or trade secrets obtained or learned as a result of my participation in the Programme, including without limitation, any information concerning or relating to the Programme, the products, the panelists, the activities contained in the Programme, the outcome of the Programme, or the contents of this Agreement or document that | sign, have signed or receive from Amway at any time.Terms & Conditions 1. The programmes opento all Malaysian ABO/APC upons ubmission of each ABO/APC's self-health background 2. The programme is scheduled to commence from 15 March 2024 to 13 May 2024. All ‘ABO programme participants must seek medical advice and not have any severe medical conditions or terminal illness and be mentally fit to participate inthis programme. 3. Although the effectiveness of the product's ingredients is backed by research studies, the result may vary for each individual, depending on the individual's health condition, 4, All ABO programme participants must consistently check their registered email during the signing up for the bootcamp, as email will be the sole tool of communication for the programme. Amway does not hold accountability if participants miss out on important communications, and appeals will not be allowed 5. Wehighly encourage all participants to actively engage in fostering apositive community environment, including on social media platforms. Therefore, Amway reserves the right to take appropriate action, including the removal of any participants who engage in negative or abusive behavior within the programme. 6. Allparticipants must undergo a health screening befor e 15 March 2024 at selected Innoquest branches prior to the commencement of the Health Warrio r challenge. Anyone who fails to do the health screening at selected Innoquest branches will be disqualified from the programme. No other health scree ning party's result will be accepted. 7. Health screening results conducted by Innoquest sinc e 1 January 2024, will be considered asthe initial reading. For thefinal reading, results obtained from 1 May 2024, onwards will be accepted. & Anyone who fails to comply with the health screening schedule as per required by the authority will be disqualified from the programme 9. Any ABO found to have provided false information will be disqualified from the programme 10. All ABO programme participants must agree to share thelr personal data and personal health data with Amway while they are enrolled in the programme for the purpose of ensuring seamless monitoring and the success of the programme. This includes the team leader and two downline participants’ health data. 11. AIl ABO programme participants are required to record their own body measurement and as per the guide below, on the FIRST and LAST DAY of the programme. Please keep photographic recordings of all measurements. These photographic recordings will serve as proof of ‘before’ and ‘after’ Allddatarecorded must be submitted on the last day of the programme, via the submission link sent to all panelists. 12. ABO programme participants are required to take photographic recordings (with white background) as per the provided guide below: ‘© Lfront view AND 1 side view (left or right) Body weight (show reading on Inbody dial/weighing scale) Body fat% Muscle mass Waist circumference (show reading of waist measurement in cm) Hip circumference (show reading of hip measurement in cm)‘TERMS AND CONDITIONS 1. The Bodykey Gut Reset Train The Trainer, also known as the Health Warrior 2024, will commence on 15 March 2024 and is scheduled to end on 13 May 2024, unless otherwise extended. However, all participants are required to abide by the terms of the contract until 15 ‘March 2025 to ensure the successful completion of the programme 2. AI|ABO programme participants must provide his/her testimonial by the end of the programme and must agree to be interviewed, photographed, videotaped, and at tend any official company-organised events and activities pertaining to this programme. All ABO programme participants may be required to share experiences from this challenge publicly during these events and activities. 3. Amway will not be held responsible in case of any ABO programme participants using the said title, attends any event not organised or approved by Amway. Further, | hold harmless and release Amway from all demands, claims and causes of action inrelation thereto. 4. When an ABO programme participant is acting on behalf of Amway, they cannot act as a representative of any LOS. 5. All ABO programme participants shall abide by the Amway Rules of Conduct. The body weight and measurement records for ‘before’ and ‘after’ can be taken using the InBodyDial or any smart weighing scale, while the waist and hip circumferences measurements can be taken using any measuring tape. However, it is compulsory for every ABO programme participants to use the SAME measuring device/tools for all the measurements for ‘before’ and ‘after’ to ensure the consistency and accuracy of the result. 14. The collected data/photographic recordings of body measurement must be submitted on the last day of the programme for verification purposes. Failure by any participants to provide their complete data/photographic recordings of ‘before’ & ‘after’ versions of themselves shall result in their being disqualified from the programme. Any late submissions will not be entertained and will not be entitled for any additional benefits if available. 15. All ABO programme participants ar e required to follow the usage guidance and instructions provided by Amway and to stay committed throughout the entire programme. 16. All ABO programme participants will be required to make their submissions via bodykeyworld.com. Anonline submission sectionwill be provided on the platform, where participants must enter all the necessary details and attach ‘before’ and ‘after’ photographic recordings. ABO participants especially leaders also encourage to submit their bi-weekly Friday task, to keep their team members engaged. 17. This submission process is crucial to ensure eligibility and avoid disqualification from the programme.As the system does not allow for amendment (ONE TIME submission only), please ensure the accuracy of all information upon submission. 18. The ABO programme participants are required to coach a minimum of 2 ABO downlines for a duration of 2 months, assisting them in submitting their results via bodykeyworld.com for monitoring purposes. Failure to adhere to the schedule of submitting the results of the 2 downlines may result in disqualification from the programme 19. All testimonials and reviews received shall belong to Amway Malaysia and Amway Brunei, and can be used in advertorials via any platform; including sales, promotions, and other publicity activities via multiple communication channels, including but restricted to digital or print. 20. Amway Malaysia and Amway Brunei reserve the right to change or overwrite the criteria, terms and conditions of this programme without any prior notice and/or in the event ofand regula ute. In the event of any dispute, uncertainty or ambiguity relating to the criteria ns hereto, or any matter involving this, the matter shall be resolved by ‘Amway Malaysia and Amway Brunei in its absolute and unfettered discretion, and the entire decision in the matter shall be conclusive and final. 21. After the successful completion of the programme on or after 19 May 2024, all ABO programme pai weight/fat reduction, muscle g pants who have achieved improvements in their health, such as |.and enhancements in health concerns like blood cholesterol, blood pressure, and blood sugar, must sustain or further improve these improvements for a minimum of 1 year. 22. Exclusion criteria of the programme: Pregnant women or those who are planning to get pregnant Persons with any other medical conditions/critical illness (e.g,, Cancer) which are not listed as above Person who needs or undergo medical practitioners' monitoring Persons who are consuming antibiotic to manage their diseases/iliness Persons who have any conditions that may impair nutrient absorption in the gut Persons with serious physical disability
