Marine Insurance Claim Procedures

This shipment has been insured on your behalf by us under policy nr. GA.10255 written by
Reaal Verzekeringen NV through the registered broker Gipon BV.
No individual policy will be issued for shipments, unless required by a letter of credit or a
governmental regulation.

This insurance is in effect from the time the insured goods leave the warehouse and continues
while the goods are in transit and/or awaiting transit until delivered to the final warehouse at
the destination. In case of deviation from the original organized voyage please inform us
directly.

Instruction in case of cargo loss or damage


In case of known or suspected loss or damage to goods insured by this policy, the following
procedure must be followed to protect your insurance claim and to assure that all of your
rights against carriers, bailees or other parties are properly preserved.

1.
Upon receipt from the carrier, all shipments should be inspected directly, with notations
being made on the delivery receipt and/or waybill as to the condition of the goods (e.g.
shortage, water damage, pilferage or damage). In the event of hidden damage, the carrier
must be given written notice as soon as the damage is discovered. In the event of a
house/house container, the seal number and container number must be noted as well as any
damage to the container. Please do take pictures of the damaged goods, the packing and the
container. Take all necessary steps to prevent further damage.
No receipt may be issued without immediate notice being given in writing of the remarks
concerning damage and/or loss noticed or suspected.

If survey is needed directly please contact yourself the nearest Lloyd’s agent or another
average agent of good repute and inform IDA Foundation . directly which surveyor has been
appointed. Ask for a survey report in the English language. No such application need be
made in case of a loss or damage under € 1.000.

The airline, steamship company, inland carrier etc, should be contacted upon notice of
damage/shortage and invite them to attend the survey. If they decline to attend the survey,
reconfirm this to them in writing.

In case of damage and/or loss immediately hold liable in writing the airline, the shipping
company the inland carrier and/or any other party who may be liable for the loss and/or
damage to the insured goods.

5.
Submit the formal claim in detail to IDA Foundation. We will pass it on to our insurance
company
a) Statement of claim detail, proving the extent and cause of the damage;
b) Original or copy of both sides of the bill(s) of lading covering the entire shipment;
c) Original or copy of the invoice(s) covering the entire shipment, illustrating terms of
sale;
d) Packing list(s);
e) Delivery receipts and any other available documents showing conditions when
received;
f) Original and duplicate of insurance policy, if issued;
g) Original survey report (in the English language) proving the extent and cause of the
damage;
h) Copy of claim against carriers or other parties and their reply, if already received;
i) Repair and/or reconditioning invoices;
j) Any other documents requested by the claims adjuster.

Upon receipt of all relevant supporting documents, our insurance company will adjust and
settle the claim.
