Hepatitis B, its prevention, and management, in order to reduce the

HOW IS THIS
PREVENTED?
• Get the HBV
vaccine. This helps
prevent the hepatitis B
infection.
• Vaccines for the
prevention of hepatitis
B have been routinely
recommended for
babies. The first dose is
generally recommended
within a day of birth
The hepatitis B vaccine
was the first vaccine
capable of preventing
cancer, specifically liver
cancer.
• Most vaccines are
given in three doses
over a course of days. A
protective response to
the vaccine is defined as
an anti-HBs antibody
concentration of at least
10 mIU/ml in the
recipient's serum. The

vaccine is more
effective in children and
95 percent of those
vaccinated have
protective levels of
antibody. This drops to
around 90% at 40 years
of age and to around 75

واليوم سنتحدث عن موضوع حظي، طالب في السنة الخامسة في كلية الطب، اسمي، صباح الخير
ونأمل في حديثنا معٍا ان نقوم، وهو التهاب الكبد الوبائي ب، باالهتمام بسبب انتشاره األخير في مصر

بتغطية دورته ،األعراض ،كيفية تشخيصه والطرق المختلفة التي يمكننا بها حماية أنفسنا لتقليل فرص
اإلصابة بالمرض.
التهاب الكبد Bهو عدوى كبدية يسببها فيروس التهاب الكبد .Bويمكن أن تكون العدوى حادة (أي شديدة وقصيرة األجل) أو
مزمنة (أي طويلة األجل).
وُيحتمل أن يسبب التهاب الكبد Bعدوى مزمنة ويزيد خطر تعرض الناس للوفاة بسبب تشمع الكبد وسرطان الكبد.
وقد تنتقل العدوى من خالل مالمسة سوائل جسم شخص مصاب بالعدوى مثل الدم واللعاب والسوائل المهبلية والسائل المنوي.
ويمكن أن تنتقل أيضًا من األم إلى طفلها.
كيف ينتقل التهاب الكبد الوبائي ب :في المناطق التي يتوطنها المرض بمعدالت عالية ،تنتقل العدوى بالتهاب الكبد Bفي أغلب
األحيان من األم إلى الطفل عند الوالدة (انتقال العدوى في الفترة المحيطة بالوالدة) أو عن طريق االنتقال األفقي للعدوى (التعرض
لدم ملَّوث) ،وال سيما من طفل مصاب بالعدوى إلى طفل غير مصاب بها خالل أول خمس سنوات من العمر .واإلصابة بالعدوى
المزمنة شائعة لدى الرضع المصابين بالعدوى عن طريق أمهاتهم أو قبل بلوغ سن الخامسة.
كما ينتشر التهاب الكبد Bبسبب اإلصابات الناجمة عن إبر الحقن ،والوشم ،وثقب الجلد ،والتعرض للدم الُم َّوث وسوائل الجسم

مثل اللعاب والسوائل الحيضية والمهبلية والمنوية .وقد تنتقل العدوى بالفيروس أيضًا عن طريق إعادة استخدام اإلبر والمحاقن
الملوثة أو األدوات الحادة في سياقات الرعاية الصحية أو السياقات المجتمعية أو لدى األشخاص الذين يتعاطون المخدرات بالحقن.

وينبغي أن تركز رعاية المصابين بالتهاب الكبد Bالحاد على الحفاظ على راحة المرضى .وينبغي للمرضى اتباع نظام غذائي صحي
وشرب كثير من السوائل للوقاية من التجفاف نتيجة للقيء واإلسهال
ويمكن عالج العدوى بالتهاب الكبد Bالمزمن باألدوية الفموية ،بما في ذلك دواء تينوفوفير أو دواء إنتيكافير.
العالج قادر على ما يلي:

وقد تنتقل العدوى بالتهاب الكبد Bمن األم إلى الطفل .ويمكن الوقاية من انتقال هذه العدوى من خالل تناول مضادات الفيروسات

لقاح التهاب الكبد الفيروسي ب هو لقاح يضمن الوقاية من عدوى التهاب الكبد الفيروسي ب .واللقاح يحتوي على واحد من
البروتينات المغلفة للفيروس ،وهو المستضد الموجود على السطح الخارجي لفيروس بي ( .)HBsAgوهي مصنعة بواسطة خاليا
فطر الخميرة ،المدرج بداخلها الشفرة الوراثية لل .HBsAgمقرر مكون من ( )3جرعات ،الثانية تؤخذ بعد شهر واحد على األقل
من الجرعة األولى والثالثة تؤخذ بعد الجرعة األولى بستة أشهر بعد ذلك ينشئ جهاز المناعة أجسام مضادة ل HBsAgفي الدم.
وتعرف األجسام المضادة ل HBsAgب anti-HBsAgهذه األجسام المضادة وذاكرة جهاز المناعة يوفر الحصانة ضد
العدوى بالتهاب الكبد الفيروسي ب .وأول لقاح أصبح متاح في عام .1981
