AFFIDAVIT OF LOSS

I, FARIDA GUIALIL KAMID, of legal age, Filipino, single and a

resident of 165 Don Rufino Alonzo Street, Cotabato City, after being
duly sworn to in accordance with law hereby depose and say that:

1. That I am a holder of the CERTIFICATE OF REGISTRATION

issued by BUREAU OF INTERNAL REVENUE (BIR);

2. That I used to put the said CERTIFICATE OF REGISTRATION

in my cabinet at our office;

3. That sometime on APRIL 2023 at the Cotabato City, I discov-

ered that my CERTIFICATE OF REGISTRATION was no longer
in my possession;

4. That, I tried my best to locate the same but my efforts were in

vain;

5. That, I consider the said CERTIFICATE OF REGISTRATION as

lost for all legal intent and purposes;

6. That, I am executing this affidavit to attest to the truth of all

the foregoing facts and to apply for the issuance of my CER-
TIFICATE OF REGISTRATION.
