It has been observed that you have not been attending work and have been absenting since
[Insert Date], without any information of absence or prior sanction of leave. You have not
even given an intimation about your absence or about your inability to attend to or attend or
resume work.

Due to your unauthorized absence, the work assigned to you has been adversely affected and
the company’s administration has been put to enormous inconvenience and loss, so also the
smooth flow of work and the work process have been disrupted, since you were assigned a
responsible work/position in the company.

You are hereby advised to resume work within 10 days hereof without fail and without any
excuse and also offer explanation for your unauthorized absence as above.

If you fail to resume work and offer satisfactory explanation, as advised herein, it will be
deemed and presumed that you are no more interested in the continuance of the employment
of the company and thus have voluntarily vacated and abandoned your employment with the
company. As a consequence, your name will be removed from the roll of the company. In
addition, you may also be dealt with under the relevant rules of employment, as the competent
authority may deem fit.

You are advised to take note of this notice, carefully.
