Project Notification Form

Note

The project notification form is split into three separate parts to allow a clear structure for
information provided by the client, the fee proposal and then the appointment process if the fee is
accepted. Upon successful appointment, a request for information will then be issued for surveys,
drawings, designer details etc. If you have any questions whilst completing the form, please contact
the office on 01869 220649.

Part 1 – is to be completed by the client or clients representative, to provide the relevant


information about the project for CSS/PHS to provide an accurate fee quote and service provision.
Please fill in all sections with as much detail as possible and attach any scoping/schedule of work
documents and a client brief if applicable.
