AFFIDAVIT OF PARENT’S CONSENT

We, ARSENIO RIMOROSA FABIAN and GINA FERRER FABIAN

and both of legal ages, Filipinos, married, and a resident of Purok 3, Cabiseria
23, San Francisco, San Antonio, City of Ilagan, Isabela, after having been duly
sworn in accordance with law, hereby depose and state that:

1. We are the biological parents of JAMES MARK FERRER FABIAN,

25 years old and was born on 27 August 1998 at Isabela Integrated
Provincial Health Office, City of Ilagan, Isabela. Attached hereto is a
copy of her Certificate of Live Birth from the PSA marked as Annex
“A”;

2. Our son, James Mark, is applying as a member of the Philippine

Army. Thus, we hereby give our full support and consent to his
application;

3. We were neither forced, coerced nor intimidated in giving this

consent;

4. We are executing this affidavit to attest the veracity of the foregoing

and for purposes of extending my consent to James Mark’s
application as a candidate soldier.
