AFFIDAVIT OF DISCREPANCY

I, CRISTELYN GUMIRAN CANCERAN, Filipino, of legal

age, single, and a resident of Purok 2, Daragutan West, San Mariano,
Isabela, after having been duly sworn to in accordance with law,
depose and state:

1. That the name appearing in my Official Transcript of Records

issued by Isabela State University, Cauayan City, Isabela and other
school records is CRISTELYN GUMIRAN CANCERAN;

2. That the name appearing in my Certificate of Live Birth is

CRISTELYN G. CANCERAN. Attached herein and marked as
Annex “A” is a copy of my Certificate of Live Birth to attest the same;

3. To remove confusion as to the two names, I hereby declare

that the name "CRISTELYN GUMIRAN CANCERAN” and
“CRISTELYN G. CANCERAN” refer to one and the same person
whose registered date of birth is March 07, 1999;

5. I am executing this Affidavit of Discrepancy for the purpose

of putting my records straight and to attest to the truth of the
foregoing facts for whatever legal purposes it may serve.
