of legal ages, Filipinos, and residents of Narra St. Purok 1, Alinguigan 2 nd, City of
Ilagan, Isabela, after having been sworn in accordance with the law hereby depose and
state that;

1. We personally know VENICE ISMAEL CAÑA, who was born on October 20,
2007 to her parents Oriel Caña and Natallie Ismael because they are our
longtime friend and neighbor at Alinguigan 2nd, City of Ilagan, Isabela, and
the facts of her birth was not registered at the Local Civil Registrar of Ilagan,
Isabela. Attached hereto is a Certification from the Office the Local Civil Registry
Office of Ilagan City, Isabela to attest the same and marked as Annex “A”;

2. We are not in any way related to any of the foregoing person within the
fourth civil degree either by consanguinity or affinity, nor do we have any
pecuniary interest in this matter whatsoever;

3. We are executing this affidavit to attest to the truthfulness of the foregoing

statements and to establish that the facts of birth of VENICE ISMAEL CAÑA
has not been registered in the Local Civil Registry of the City of Ilagan, Isabela
for all legal intents and purposes it may serve.
