AFFIDAVIT OF DISCREPANCY

I, GILLY ROSE GAÑGAN SANCHEZ, Filipino, of legal age,

married, and a resident of San Ignacio, City of Ilagan, Isabela, after
having been duly sworn to in accordance with law, depose and state:

1. That the date of birth appearing in my Professional

Regulatory Commission (PRC) ID with Registration No. 1296544 is
October 30, 1991;

2. That the date of birth appearing in my Certificate of Live

Birth is October 31, 1991. Attached herein and marked as Annex “A”
is a copy of my Certificate of Live Birth to attest the same;

3. That my true and correct date of birth is October 31, 1991 and
not October 30, 1991 as erroneously appearing in my PRC ID;

4. That I have been using my date of birth as October 31, 1991 in

all my personal and official documents;

5. I am executing this Affidavit of Discrepancy for the purpose

of putting my records straight, to renew my PRC ID, and to attest to
the truth of the foregoing facts for whatever legal purposes it may
serve.
