RE: CIRCUMSTANCE OF SOLO PARENT

We, ATANACIO PADDAYUMAN MADDARA II and JAYSON

TAGUIAM MAGUNDAYAO, both of legal age, Filipinos, both residents of
Brgy. San Ignacio, City of Ilagan, Isabela, after having been sworn in accordance
with law, hereby depose and state that:

1. We know FREDO B. CAGUIAT because we are neighbors in Brgy. San

Ignacio, City of Ilagan, Isabela for more than thirty (30) years now;

2. We know that he is a SOLO PARENT and he was separated from her wife for
more than three (3) years now;

3. Hence, he was left with the sole responsibility of parenthood and he is not
cohabiting with a partner or co-parent;

4. That we are not related to FREDO B. CAGUIAT by consanguinity or affinity;

5. We are executing this affidavit to attest to the truth of the foregoing and for
whatever legal purpose it may serve.
