Work placement information


Work placement provides you with an opportunity to learn and be assessed in a hands-on, real-life
industry environment. Learning in the classroom is important, and you will engage in lots of classroom
discussion, practice and be involved in demonstrations and simulations before you are ready for your
work placement – but gaining practical experience during your work placement hours provides you
with the opportunity to put your new-found knowledge into practice and provides you with an
appropriate environment to be assessed in. It also provides you with opportunities to observe qualified
and experienced community services workers and allows you to learn while under the close
supervision and guidance of a workplace supervisor and mentor.
You will not be paid for your work placement as it forms part of your training and assessment for this
qualification.

Responsibilities
You are always expected to comply with all of your work placement’s policies and procedures for their
staff members and follow your supervisor’s instructions. You will receive the required information
about working hours, dress requirements and specific guidelines from your work placement before
you begin and will sign a work placement agreement prior to starting. If you’re unsure of anything then
speak to your trainer/assessor and ask questions when the placement arrangements are being made.
You may undertake more than one placement at a different facility – this will all be arranged with your
trainer/assessor and made clear to you during this time.
Your work placement supervisor will arrange an induction for you where the expectations will be
outlined and the limitations of what you are able to do while you are at your work placement.

Jessica Robert frequently experiences memory loss, particularly concerning short-term memory. She often struggles to recall recent

Describe the process that you undertook to obtain this person’s informed consent to participate in your assessment task.

I provided Jessica Robert with an introduction, explained the assessment task, assured confidentiality, emphasized voluntary

participation, obtained her consent, and documented the procedure to ensure her informed consent.

I consulted with Jessica Robert's primary caregiver, reviewed the care plan documented at the facility, and noted any specific requests
or preferences to familiarize myself with her individualized plan.

Describe the process that you undertook to familiarise yourself with this person’s specific needs and wants. How were these needs and wants reflected in the
individualised plan? Include a description of the person’s needs for a stable and familiar environment and what that looks like.

I engaged in open discussions with Jessica Robert, her family, and the care team to gain insight into her specific requirements and
preferences. I observed her daily routine, preferences, and any challenges she faced. Additionally, I reviewed her medical history,
dietary needs, medication schedule, and specified activities outlined in her personalized care plan. The plan prioritized providing a
stable and familiar environment for Jessica by maintaining consistent routines, familiar caregivers, and a comfortable living space to
minimize anxiety and confusion. This included ensuring her room setup remained unchanged, providing familiar items, and
fostering a tranquil and supportive atmosphere within the care facility.

Describe the process that you used to access information about the person’s reminiscences and routines. Was this information accurately reflected in their
individual plan?

I communicated with Jessica Robert and her family to learn about her past experiences, interests, and important life events to gather

insights into her memories and routines. By observing her interactions and daily activities, I gained an understanding of her current
habits and preferences. Additionally, I reviewed existing records, such as life histories or memorabilia, which contained significant
details about her life story. Jessica's personalized plan, which incorporated information about her favorite activities, past interests, and
meaningful experiences, was informed by this knowledge. To ensure her care and support were tailored to her specific needs and
preferences, the plan documented her routines, preferences, and memories.
