1. I, WILSY T GERBOLINGA, of legal age, married, Filipino and a residence of

B59 L27 Heroesville 2 Barangay Pinugay, Baras, Rizal, Philippines, after having been
sworn in accordance with the law, depose and state;

2. That I am one of the investor and member of the Organic Home Wellness under
Great 4 International Marketing Corporation located at G-16 Regalla Park Towers, P.
Tuazon Brgy Socorro, Cubao, Quezon City;
3. That I am one of the product distributor and marketing sales members of the said
business;
4. That said business gain profits through networking by selling Motorcycle, soap,
shampoo coffee, herbal medicines and other health products;
5. That my main job function is to recruit member, direct referral, product
sales/distributions and market a company to different stakeholder through social Medias
platform;
6. That I am enclosing the following documents to prove my claims;
a. Receipt/ Invoice;
b. Transmittal;
c. Cheques;
d. Payout schedule PRP rewards

7. That I am executing this affidavit as part of the documentary requirements to

support my application for tourist visa and to attest the truth and veracity of the foregoing
facts and circumstances for whatever legal purpose it may serve.
