Allergan EHS Risk Assessment This Program establishes the minimum requirements for conducting and documenting EHS

risk assessments. The purpose or the analyses is to ensure that occupational, process and environmental hazards and risks are known and controlled at each Allergan facility. EHS risk assessments include occupational exposure assessments, industrial hygiene assessments, job hazard analysis, environmental and process safety assessments. The outcome of these assessments will result in an overall aspect and impact summary for the facility in question. This allows the facility to develop corrective and preventive action plans for each significant aspect or risk. Significance is determined by the risk rating which is derived using the Allergan risk assessment template and multiplying the frequency of occurrence times likelihood times the severity. Allergan has established risk ranking categories of I through IV, IV being the most significant risks and I being the least significant risks. The work place exposure assessments are oriented to biological, chemical or physical risks associated with the facilitys operations. An exposure assessment plan will be developed if significant risk is identified for the particular operations. Industrial hygiene assessments include anticipating, recognizing, evaluating, and controlling those environmental factors and stresses arising in or from the work place that may cause sickness, impaired health or well-being, or discomfort among employees or members of the community. The job hazard analysis promotes and facilitates proactive job hazard analysis and provides a mechanism for analyzing, identifying, and prioritizing opportunities for risk reduction. The basic elements of the system include: Task identification and specification Hazards identification for specified tasks Risk analysis and rating of identified hazards Identification of existing controls used to reduce overall hazard risks Identification of additional controls which could further reduce the hazard rating This system is being implemented within Allergan to enhance proactive occupational health and safety risk reduction efforts and to provide a means to identify and implement injury and illness prevention measures. Once the risk are identified and significance is determined, a set of corrective or preventive actions must be developed to eliminate or reduce the risk. Communications to employees of the risk and the plans to correct are required.
