Schools Division Superintendent
E. Quirino Avenue, Davao City

Thru: KATHERINE ANNE C. QUIMSING

State Auditor IV, Audit Team Leader
Commission on Audit (COA)

Sir:

I am writing this explanation letter in reference to the late submission of the

Documentary Requirements on Request for Relief from Money and Property
Accountability to the office of the Commission on Audit.

On July 17, 2021, Diego Silang Elementary School was set ablaze by a nearby
house on fire. As the former school property custodian, I submitted the request for
relief of accountability on August 13, 2021, in hopes of notifying your good office
not later than 30 days. However, due to the COVID-19 pandemic, it reached your
office late. I wasn’t allowed to hand-carry it because a drop-box system was
implemented.

Attached herewith are Division Memorandum 101, s. 201, and the received
Doctrack ticket.

I am hoping for your kind consideration on this matter. Thank you.

Very truly yours,

DAISY G. ANTIMOR
Teacher I
